Restrictions and limitations: Family members must be immediate family members of the membership plan holder. Restoration filling coverage is limited to 1 tooth per member in the plan. Children of account holder must not be older than 18. Please note – although each of the services offered with the membership plan are limited to one application per member per year, referral rewards and 10% discounts will still be available for the duration of the membership plan year.
